I don't know about other forms of fast-food delivery (besides chinese, I don't know of any... although I have heard McDonalds and others do delivery in some urban areas), but when it comes to pizza I usually use this site:

Its' not necessary so I think because that's there job they get payed to do that but If I happen to get good service and have an extra buck or to then I tip but when they stand there and expect a tip I don't give them **** why should I? uhh if you go to fast food like Mcdonalds or Wendy's then you probably will not pay very much for the meal so the tip with only be like 17 cents so dont bother ahahah
Minimum of 10%....more if they are really fast.
